What is the Planning Ahead tool?

Provided by Hospice UK, this tool explains the kinds of decisions many people wish they had prepared for, and it gives you various options to think about.


The LifeBook is a free booklet where you can write important and useful information about your life, from who insures your car to where you put the TV licence.

The LifeBook won't only help you to be more organised, but could also be invaluable to a family member or a friend if they need to find important information about you in an emergency.

A workbook compiled by the Kirklees and Calderdale End of Life Care Education Group, sharing best practice around end of life care. 

It helps care and support staff to recognise what good end of life care looks like, in order to make a difference. 

Working in communities to support each person to experience the end of life and death that they choose alongside their friends and family.

Dying Matters is a national campaign hosted by Hospice UK. They work in partnership with grassroots communities around the UK and supports a week of events annually in May that aim to get people talking and sharing stories openly about dying and grief, to reduce the associated stress, stigma and social isolation that can accompany end of life experiences and long-term caregiving, bereavement and grief.
